NEW IDENTIFICATION CARD INSTRUCTIONS
ALL DIVISION 2 MEMBERS
(Watch this space for future dates and times.)
All Division 2 members are invited to participate.
All auxiliarists must complete the
ID Card Application (~20KB PDF) on site or beforehand.
This information will be used to complete the backside of
the ID Card.
A digital picture will be taken of the Auxiliarist in
uniform. The prepared ID Cards will then be sent to DIRAUX
for signature and will be sent back to Division 2 in
approximately two to three weeks. After lamination the card
will be forwarded to the appropriate FC for distribution.
SECURITY PROCESSING INSTRUCTIONS
ALL MEMBERS
Any member not already having done so should immediately print
and complete one (1)
Personal Security Questionnaire (PDF) and mail to:

FOR OPERATIONAL SUPPORT PERSONNEL
If you participate in Operational Support, meaning you DO
NOT participate in Direct Operations (e.g. coxswain, crew,
watchstanding, etc.) you may immediately complete the
following four (4) steps:
STEP 1.
Print & Complete one (1)
OFI Form 86C Special Agreement Check (PDF)
Notes for Form OFI 86C:
- Do not fill out questions 7, 8, 9, 10, 11, 12 & 14.
- Native Born American Citizens fill out 1 thru 6, 13a & 13b.
- Naturalized American Citizens fill out 1 thru 6, 13a, 13b, & 13c.
- Dual Citizens fill out 1 thru 6, 13a, 13b, 13c, 13d.
- If you are not an American Citizen, please contact your Flotilla Commander.
STEP 2.
Print & Complete one (1)
Form SF-85 (Authorization for Release of Information)

Notes for Form SF-85:
- The above PDF document only contains Page 6 of the
entire SF-85 form
- Operational Support personnel only need to fill out this
page of the form
STEP 3.
Get fingerprinted on three (3) sets of Fingerprint Cards
(provided to you at time of fingerprinting)
Notes for Fingerprint Cards:
- You must be fingerprinted by a certified member of the Auxiliary.
- Your FC will advise when and where fingerprinting will be available.
STEP 4.
Send all of the above in one (1) package to:

FOR DIRECT OPERATIONS PERSONNEL
If you participate in Direct Operations
(e.g., coxswain, crew, watchstanding, etc.) you will be
contacted when to submit your forms and should NOT sign, date
or send them in until directed. This could occur anytime
between now and September 2006 depending on district quotas.
In the interim or when directed you may:
STEP 1.
Download and complete the 13-page SF-86
Form which can be obtained along with instructions from: www.uscgaux.org/~forms/sec_test.html
If completed in advance you will have to
review for currency and update if necessary when directed to
one (1) original and one (1) copy of the form, each with an
original signature. If you have the ability to save PDF
files to your computer, the fill-in capability of the SF-86
makes completing all form fields very easy.
STEP 2.
Print and complete the one-page
DHS 11000-9 Form (Disclosure and Authorization Pertaining to
Consumer Reports Pursuant to the Fair Credit Reporting Act)
(PDF).
STEP 3.
Get fingerprinted on three (3) sets of Fingerprint Cards
(provided to you at time of fingerprinting)

STEP 4.
Send all of the above in one (1) package to:
